The YWCA Greater Harrisburg is seeking a full-time Supportive Services for Veteran Families (SSVF) Case Manager. This position is provides time-limited Housing Supportive Services for very low-income Veteran households and families who are homeless and about to enter permanent housing within 90 days; have left housing to access permanent housing more suitable to their needs within 90 days; and/or are at immediate risk of becoming homeless.
Case management services involve coordination and assistance in housing search, placement, and permanent housing retention; access to Veteran and mainstream benefits; overcoming housing self-sufficiency barriers; and providing limited housing rent, utility, emergency housing and housing stabilization assistance for Veterans in the Greater Harrisburg Region of Dauphin, Cumberland, Lebanon, Perry and York counties.

Position Details:
- Hours: 37.5 hour work week; Monday-Friday 8am-4pm; with a rotational on-call schedule.
- Salary: $18.31/hr.
- Outreach to very low-income Veteran households, including homeless, both unsheltered and in emergency shelters, and at places where they congregate.
- Interview and assess for SSVF eligibility using Eligibility Screening Tool.
- Assess and prioritize supportive services based on housing and needs assessment
- Utilize Evidence Based Practices of Housing First, Motivational Interviewing and Critical Time Intervention in case management
- Create Individual Goal Plan with client for permanent housing stability.
- Function as a multi-disciplinary team with the YWCA’s Veterans Services Dept.
- Assist clients with housing search, placement, or prevention of loss of housing.
- Inspect potential housing units for housing codes, safety, and habitation standards.
- Serve to mediate concerns involving the landlord/property manager, participant, and participant household members to ensure housing stability.
- Collaborate with existing landlord/property managers to identify any concerns relating to housing site safety, ongoing unit availability, and other factors that may impact ongoing access to safe and affordable housing.
- Schedule and conduct weekly individual case management.
- Provide, coordinate SSVF temporary financial assistance e.g. rent, utility, security deposit assistance following SSVF temporary financial allocation and limitations.
- Coordinate with the Lebanon VAMC for outreach, referral, and join case planning so that veterans will receive full VA benefits and mainstream benefits.
- Coordinate with other Veteran Services Organizations.
- Coordinate with the Continuum of Care networks for CoC PA501, PA507, and 512
- Bachelor's degree in human services or related field with prior related work experience. An equivalent combination of education and experience will also be considered.
- In-depth knowledge and understanding in social services, veteran benefits, and other supportive services.
- Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required.
